  • Hospitalization for congestive heart failure and discharged within the past 30 days
  • At least 18 years of age
  • Been treated for heart failure in a hospital in the past 30 days
  • Type 1 diabetes
  • Severe Kidney disease
  • Cannot be currently treated with SGLT2i such as: Farxiga, Invokana or Jardiance
  • Any upcoming surgeries
