Cardiovascular Disease

You may qualify if you meet the following requirements:

• 18 years of age or older
• Have a history of cardiovascular disease
• Have elevated cholesterol levels:

OR

1 of the following

◦ Recent heart attack (about 3 months ago to less than 1 year);

◦ Type 2 diabetes;

• Taking medication(s) to lower cholesterol (if applicable).

◦ You may be eligible if you are taking a low dose of statin medication, no statin medication (due to intolerance), or a combination of  cholesterol-lowering medications.
